The widespread idea that orthodontic treatments with premolar extraction damage profile and facial harmony often discourages the orthodontists to choose this treatment plan. However, the literature denied this statement, since patient initial features and biomechanics can determine the extent of treatment changes. Extraction orthodontic treatment can solve space, periodontal and esthetic problems; these improvements could affect all these aspects or only some. Several diagnostic factors during case planning allow to predict the extraction treatment effects on the final outcome. The choice of the ideal treatment plan must be tailored to each individual patient and an accurate diagnosis is essential through dental, periodontal and esthetic analysis. The appliance efficiency is equally a key factor to achieve the set treatment goals.
