Orthodontic Pre-Surgical Protocols for Double Jaw Surgery

Bimaxillary surgery is the most common procedure in orthognathic surgery since it allows a greater potential of 3D repositioning of the jaws. In particular Bimaxillary surgery allows an effective resolution of the transverse discrepancies using a 3 piece Le Fort, a correction of biretrusive conditions in Class 2 and Class 3 skeletal malocclusions, the possibility of rotating the occlusal plane, a more efficient control of a vertical defect and excess (short and long faces), a 3D correction of skeletal asymmetries. A bimaxillary surgery requires an effective coordination between the orthodontist and the surgeon. The 3D surgical repositioning of the jaws is possible when the orthodontic preparation of the arches respects specific requirements. We will propose protocols of orthodontic pre surgical preparations to be applied in cases of mandibular advancement with short faces, significant deep bite and collapsed C. Of Spee. We will focus of the proper positioning of upper and lower incisors in cases of bimaxillary advancement and Clock Wise (CW) rotation of the occlusal plane. We will describe how to position the incisors in case of CCW rotation of the occlusal plane.
